Service Center Helpful hints:
ADULT REGISTRATION FORMS require TWO signatures of approval from the sponsoring organization. Social Security number is REQUIRED on all adult registration forms.
TOUR PERMITS require TWO signatures. Vehicle insurance information requested must appear on the tour permit form. All tour permits submitted for approval must be on a 2005 printing date or later.
