Medical Form (included in Leader's Guide)
The Great Salt Lake Council and Teton High Adventure Base (THAB) places the safety of the program participants as the highest priority. All participants must be at least 13 by January of the year they attend. National High Adventure standards require each participant (youth and adult) to complete a yearly physical exam using the Class 3 Medical Record Form (514 Kb).

The Teton High Adventure Base is located approximately 1 mile south of Hoback Junction on Highway 89 near Jackson, Wyoming. Allow a minimum of five hours travel time from Salt Lake City. Map from Salt Lake City (161 Kb)
